Please note the song is actually 5/4 and not 4/4. Where there is an eighth note immediately played twice, it should only be punched out only once. You can easily see this in the editor as an orange dot followed by a red dot. Just punch a hole in between the orange and red note.

To represent "acciaccatura" in the editor, I had to depict this as a note played immediately twice. If you clone this tune and open in the editor you will see the first note in orange and the next in red. In this situation, do not punch the orange note and simply punch the red note slightly offset to the left of its original placement. I'd recommend the offset to be half the radius of the hole punch. In the situation where there is an orange note immediately followed by two red notes, punch the orange note, do not punch the first red note and punch the second red note offset to the left of its original location. Again offset should be half the radius of the hole punch. Alternatively, you can ignore the red notes altogether as acciaccatura is meant to be played quickly before the essential note. I'm not particularly happy with my transcription of the ending at measure 74 to 81.

The notes of each chord should be punched so that timing is faster than what I have shown. The editor won't allow for notes to be punched in between consecutive vertical lines. I found that if you spaced the notes by 1/3 between vertical lines, it generates a suitable sound. For the very last chord, use 1/2 spacing between vertical lines.

The individual notes in the last chord (bar 18) should be separated in time evenly. The editor doesn't allow fractions, but I found it sounds okay if I space three notes between vertical lines.
